	<p>
	ตามพระราชบัญญัติชื่อ พ.ศ. 2505 ปรับปรุง พ.ศ. 2530  มาตรา 13  หญิงไทยทุกคนต้องกลับมาใช้นามสกุลเดิมหลังการหย่า
	</p>
	<p><b>
	ก. การแจ้งเปลี่ยนนามสกุลหลังการหย่าชั่วคราวในหนังสือเดินทาง</b>
	</p>
	<p>
	สามารถทำได้แต่ในหนังสือเดินทางรูปแบบเก่าเท่านั้น ในหนังสือเดินทางอิเล็กทรอนิกส์ (อี-พาสปอร์ต) ไม่สามารถขอแจ้งเปลี่ยนได้แต่อย่างใด
	</p>
	<p>หากท่านยังถือหนังสือทางรูปแบบเก่าอยู่ (ไม่ใช่ อี-พาสปอร์ต) ท่านสามารถมายื่นคำร้องขอเปลี่ยนนามสกุลหลังการหย่าชั่วคราวในหนังสือเดินทางด้วยตัวเองหรือทางไปรษณีย์ได้
	โดยใช้เอกสารประกอบดังนี้ 

	</p>
	<ul>
	<li>สำเนาของคำพิพากษาหย่าที่มีผลบังคับใช้  1 ชุด
	<li>หนังสือเดินทางตัวจริง และสำเนาหนังสือเดินทาง 1 ชุด
	<li>สำเนาทะเบียนบ้านไทย หรือ บัตรประจำตัวประชาชน 1 ชุด
	<li>ซองเปล่าติดแสตมป์ 4 ยูโร
	<li>ค่าธรรมเนียม 3 ยูโร (ส่งเป็นเงินสดเท่านั้น)
	</li>
	</li>
	</ul>
      </p>
	
	<p>
	<b>ข. การแจ้งบันทึกฐานะทางครอบครัวหลังการหย่า (คร. 22) ในประเทศไทย</b>
	</p>
	<p>
	<ol>
	<li>นำคำพิพากษาหย่าที่มีผลบังคับใช้แล้ว ไปให้ล่ามที่มีใบอนุญาตจากศาลแปลเป็นภาษาไทย 
	และมอบสำเนาบัตรประชาชนหรือทะเบียนบ้านให้ล่ามด้วย 1 ชุด  เพื่อให้ล่ามแปลชื่อ-นามสกุลของท่านให้ตรงกับบัตรประจำตัวประชาชน 
	ล่ามต้องเย็บคำแปลภาษาไทยและต้นฉบับภาษาเยอรมันที่เป็นก็อปปี้  เข้าด้วยกันเป็น 1 ชุด พร้อมประทับตราและลงลายมือชื่อรับรองการแปลด้วย

	<li>นำคำพิพากษาหย่าและคำแปล (เอกสารจากข้อ 1) มารับรองที่สถานกงสุลใหญ่ ฯ (ค่ารับรองเอกสาร 30 ยูโร)

	<li>นำคำพิพากษาหย่าและคำแปล (เอกสารจากข้อ 2) ที่รับรองมาจากสถานกงสุลใหญ่ ฯ ไปรับรองครั้งสุดท้ายที่ 
	กองสัญชาติและนิติกรณ์ กรมการกงสุล กระทรวงการต่างประเทศ  ถนนแจ้งวัฒนะ เขตหลักสี่ กรุงเทพ ฯ 10210

	<li>นำคำพิพากษาหย่าและคำแปลที่รับรองจากกระทรวงการต่างประเทศ (เอกสารจากข้อ 3) 
	ไปแจ้งนายทะเบียนที่เขต/อำเภอ ที่ท่านมีชื่อในทะเบียนบ้าน เพื่อขอให้บันทึกฐานะแห่งครอบครัวหลังการหย่าในประเทศไทย (คร. 22)  
	และขอเปลี่ยนนามสกุลในทะเบียนบ้านภายหลังการสมรสตามสามีต่อไป
	หลังจากนั้นจึงนำทะเบียนบ้านไปติดต่อขอทำบัตรประจำตัวใหม่ 
	<p></p><b>บัตรประจำตัวประชาชนต้องไปทำด้วยตัวเองที่ประเทศไทยเท่านั้น</b> ส่วนการแจ้งบันทึกฐานะทางครอบครัวหลังการหย่า 
	สามารถมอบอำนาจให้ญาติพี่น้องไปดำเนินการให้ได้
