						<b>หนังสือสำคัญประจำตัวซีไอเพื่อเดินทางกลับประเทศไทย (Certificate of Identity)</b>
						</p>


	
	
	     <p>
		หนังสือสำคัญประจำตัวซีไอเพื่อเดินทางกลับประเทศไทย (ใบขาว) จะมีอายุนับตั้งแต่วันที่ออกเพียง 1 เดือน
		ออกให้เพื่อเดินทางกลับประเทศไทยเท่านั้น ผู้ถือหนังสือสำคัญประจำตัวซีไอนี้จะไม่สามารถเดินทางไปประเทศที่สามได้แต่อย่างใด
	</p>
         <p>
		สถานกงสุลใหญ่ ฯ จะออกหนังสือประจำตัวซีไอเพื่อเดินทางกลับประเทศไทย ด้วยเหตุผลต่อไปนี้
	     <p>
	       <ul style="list-style-type:disc">
		<li>ผู้ขอมีเหตุผลจำเป็นต้องเดินทางกลับประเทศไทยอย่างกระทันหัน แต่ไม่สามารถขอหนังสือเดินทางเล่มใหม่ได้ทันเวลา
		ในกรณีนี้ ผู้ขอต้องยื่นบัตรโดยสารเครื่องบินเพิ่มเติมด้วย
		<li>ได้รับการขอจากทางการเยอรมัน เพื่อส่งบุคคลสัญชาติไทยที่กระทำผิดต่อกฎหมายเยอรมันให้เดินทางออกนอกประเทศเยอรมนี
		
		</li>
		</ul>
	      </p>
		<p>
		ผู้ขอบัตรประจำตัวซีไอต้องเขียน<a href="http://www.thaigeneralkonsulat.de/de/form/reisepass.pdf">คำร้องหนังสือเดินทาง</a> 
		2 ชุด กรอกข้อความให้ครบทุกช่อง</b> 
		(ดู <a href="http://www.thaigeneralkonsulat.de/th/consular/ausfuellen.html">"คำแนะนำในการกรอกคำร้อง"</a> และ 
		<a href="http://www.thaigeneralkonsulat.de/de/form/PPMuster.pdf">"ตัวอย่างการกรอกคำร้อง"</a>)
		พร้อมแนบเอกสารต่อไปนี้

		<li>สำเนาหนังสือเดินทาง 2 ชุด และหนังสือเดินทางตัวจริง (หากมี)
		<li>สำเนาทะเบียนบ้านไทย หรือ สำเนาบัตรประจำตัวประชาชนไทย 2 ชุด		
		<li>รูปถ่ายสีหน้าตรงขนาด 2 นิ้ว จำนวน 4 รูป (ถ่ายไว้ไม่เกิน 6 เดือน หน้าตรง ไม่ยิ้ม ไม่สวมหมวก ไม่สวมแว่นตาดำ ใส่ชุดสุภาพ)  
		<li>บัตรโดยสารเครื่องบิน พร้อมสำเนา 2 ชุด และ
		<li>ต้องกรอก<a href="http://www.thaigeneralkonsulat.de/de/form/befragung_ci.pdf">บันทึกการสอบสวนประกอบการออกหนังสือสำคัญประจำตัวซีไอ</a> 2 ชุด
		แนบประกอบคำร้อง
	  </li>	
	</ul>
		     
      </p>
       <p>
		อย่างไรก็ตาม สถานกงสุลใหญ่ ฯ ขอแนะนำให้ผู้ที่ประสงค์ขอบัตรประจำตัวซีไอโทรศัพท์สอบถามขอข้อมูลเพิ่มเติมก่อน
		เนื่องจากความจำเป็นในแต่กรณีแตกต่างกันได้
