						<b>คำแนะนำในการกรอกคำร้องหนังสือเดินทางรูปแบบเดิม</b>
						</p>
    <p>
	ในการกรอกคำร้อง ขอให้ปฏิบัติดังนี้ (ดู  <a href="http://www.thaigeneralkonsulat.de/de/form/PPMuster.pdf" class="sublink1"><span class="sublink1">
	    "ตัวอย่างการกรอกคำร้อง"</span></a>)
	</p>
	<ul>
	<li>ถ้ายังไม่เปลี่ยนนามสกุลหลังสมรส ให้กรอกชื่อและนามสกุลตามทะเบียนบ้าน 
	หรือบัตรประจำตัวประชาชน และเซ็นชื่อตามทะเบียนบ้าน หรือบัตรประจำตัวประชาชน
	<p></p>
	<b>โปรดทราบ</b> หากท่านยังไม่ได้เปลี่ยนนามสกุลหลังสมรสตามสามีในทะเบียนบ้านไทย
	ท่านจะได้รับหนังสือเดินทางเล่มใหม่ตามชื่อที่ปรากฎในทะเบียนบ้านไทยเท่านั้น คือ ชื่อเดิมก่อนการสมรส ดังนั้น 
	ท่านควรรีบเปลี่ยนนามสกุลตามสามีในทะเบียนบ้านไทย ก่อนมายื่นขอหนังสือเดินทาง อี-พาสปอร์ต เล่มใหม่ 
	<p></p>
	นามสกุลที่ท่านเปลี่ยนในหนังสือเดินทางก่อนนั้นจะมีอายุใช้งานตามอายุของหนังสือเดินทางเท่านั้นและยังไม่สมบูรณ์ตามกฎหมายไทยแต่อย่างใด  
	
	<p></p>
	<li><b>มุมขวาบนสุดของคำร้อง ในช่องสี่เหลี่ยม 2 ช่อง</b> ให้เซ็นชื่อและนามสกุลตามทะเบียนบ้านไทยเป็นภาษาไทยหรืออังกฤษ 
	ในแต่ละช่องให้มีทั้งชื่อและนามสกุลในช่องเดียวกัน
	<p></p>
	<li><b>มุมขวาล่างสุดใต้กรอบรูป</b>ให้เซ็นชื่อตามทะเบียนบ้านเป็นภาษาไทย
	<p></p>
	<li><b>บรรทัดที่ 7 ช่อง "ตั้งบ้านเรือนอยู่ที่"</b> ให้กรอกที่อยู่ในประเทศเยอรมนี และเบอร์โทรศัพท์ที่ติดต่อได้  
	เขียนที่อยู่ให้สมบูรณ์และชัดเจน เพื่อไปรษณีย์จะได้ส่งจดหมายได้ถึงที่ หน้าตู้ไปรษณีย์ต้องมีชื่อตัวเองและชื่อสามีติดอยู่  

	<p></p>
	<li><b>บรรทัดที่ 13 "บุคคลใกล้ชิด"</b> ให้กรอกชื่อ ที่อยู่ และหมายเลขโทรศัพท์ของคนใกล้ชิด  2  คน (พ่อแม่ หรือ พี่น้อง หรือ ญาติ หรือ เพื่อน)
	ถ้าเป็นไปได้ ในเมืองไทย 1 คน และในเยอรมนี 1 คน กรุณาเขียนชื่อและที่อยู่ให้ชัดเจน 
	ชื่อและที่อยู่ของบุคคลใกล้ชิดนี้ มีความสำคัญมาก เพราะเมื่อท่านทุกข์ยาก/ลำบาก/เจ็บ/ตาย 
	ทางราชการจะสามารถติดต่อบุคคลใกล้ชิดของท่านได้ 
 
	<p></p>
	<li>หน้าหลังของคำร้องหนังสือเดินทาง ซึ่งเป็น "สัญญายินยอมชดใช้ค่าเสียหายของผู้ที่จะเดินทางไปต่างประเทศ" 
	ให้กรอกที่อยู่ในประเทศเยอรมนี และเซ็นชื่อเป็นภาษาไทย
	</li>
	</ul>
      </p>
	
	<p>
	<b>สำหรับหนังสือเดินทางเด็ก มีข้อแนะนำเพิ่มเติมดังนี้</b> 
	</p>
	<ul>
	<li>ให้กรอกคำร้องโดยใช้ชื่อลูก  ถ้าลูกยังเขียนหนังสือไม่ได้ ให้พิมพ์นิ้วหัวแม่มือซ้ายของลูกแทนการเซ็นชื่อ 
	ที่ช่องสี่เหลี่ยมมุมขวาบนสุด ช่องละ 2  ครั้ง และใต้รูป 1  ครั้ง กรุณาใช้<b>แป้นหมึกสีน้ำเงิน</b>หรือ<b>สีดำ</b>เท่านั้น
	<p></p>
	<li>หน้าหลังของคำร้องหนังสือเดินทาง ซึ่งเป็น "สัญญายินยอมชดใช้ค่าเสียหายของผู้ที่จะเดินทางไปต่างประเทศ"
	ให้ใส่ที่อยู่ของลูกในประเทศเยอรมนี ให้พิมพ์นิ้วหัวแม่มือซ้ายเหมือนกัน ให้พ่อแม่ 
	หรือ ผู้ปกครองตามกฎหมายลงชื่อในช่องผู้ยินยอมด้วย
